Células madre en la regeneración de tejidos: avances, aplicaciones clínicas y perspectivas biotecnológicas

Las células madre constituyeron uno de los recursos más relevantes en la medicina regenerativa contemporánea, caracterizadas por su capacidad de autorrenovación y diferenciación hacia múltiples linajes celulares, lo que las posicionó como candidatas terapéuticas para la restauración de tejidos lesionados. El objetivo general de esta revisión narrativa fue analizar el estado actual del uso de células madre en la regeneración de tejidos e integró perspectivas clínicas y biotecnológicas para identificar avances, limitaciones y oportunidades terapéuticas. Se realizó una búsqueda bibliográfica en PubMed, Scopus y Google Scholar con términos relacionados con células madre, ingeniería de tejidos y medicina regenerativa, y se seleccionaron 25 fuentes publicadas entre 2015 y 2026. Se examinaron los principales tipos celulares (mesenquimales, pluripotentes inducidas y embrionarias) y las técnicas biotecnológicas contemporáneas de ingeniería de tejidos (scaffolds, hidrogeles y bioimpresión 3D). Se analizaron las aplicaciones clínicas en ortopedia, cardiología, neurología y dermatología, y se identificaron los principales desafíos éticos, técnicos y regulatorios que condicionaron su implementación. Los resultados principales mostraron que: (i) las iPSC y la bioimpresión 3D representaron los avances más prometedores del campo; (ii) las células madre mesenquimales acumularon la mayor evidencia clínica, con reducciones significativas del dolor y mejoría funcional a 12–24 meses en osteoartritis de rodilla; (iii) el metaanálisis ACCRUE no demostró beneficio significativo global de la terapia celular post-infarto sobre la fracción de eyección, pero identificó subgrupos con posible efecto beneficioso; (iv) el trasplante de iPSC con compatibilidad del MHC mejoró el injerto neuronal en modelos preclínicos de enfermedad de Parkinson sin formación tumoral; y (v) la terapia basada en exosomas y vesículas extracelulares surgió como alternativa acelular con propiedades cardioprotectoras. Finalmente, persistieron brechas metodológicas, regulatorias y de accesibilidad que limitaron la traslación clínica rutinaria de estas terapias.
